How do I setup the appointment?

To set up an appointment time please call the hospital when you are ready. If you would like a specific doctor to perform the procedure please let us know during the phone call. We recommend that the appointment be either first or last of the day so the hospital is not overly busy for more privacy. We also have a separate comfort room and entrance which we have decorated to have more of a home feeling with a couch and soft music playing.

Make an Appointment
We are happy to book an appointment for you. This is not an automatic process. Once we receive your request, we will contact you to confirm a date and time that is as close to your request as possible.

This form should only be used for requesting appointments that will take place at least 2 full business days after the time of the submission of this form.

If your appointment has not been confirmed within 24 hours, please feel free to contact the practice by phone to confirm.
